By submitting your intake form you agree that you have read and understand our privacy policies as follows:

You understand that Deborah Stauss is a Naturopathic Doctor and NOT a medical doctor and as such cannot diagnose conditions or prescribe medications. She will never recommend going off your prescribed medications but she may recommend that you  consult your physician concerning such meds if she feels there is a concern. 

Deborah Stauss, ND may give suggestion on what you can do to better your health, including exercise, supplement concerns, and other natural therapies.

You agree to not hold Deborah Stauss, ND or New Life Spa responsible for any decisions you make concerning your health.

Your information will be kept private and only used in case of an emergency if needed.

You agree to consultations via Zoom and Telephone, whichever will work best for you.
